Requirements
To purchase a product available online by pre-paid Company Check, print the appropriate Order Form below. Fill in the required information and fax your order to 514-392-9120. An invoice will be issued. Upon receipt of your check, the product will be shipped to you at the Shipping Address designated in the Order Form. Please note that Fedex will NOT deliver to a PO Box.
